There is a report in UORares.com; it's a event object and left unlocked. A player picked it up but GM tried to retrieve it from him.
Finally, GM took a copy of it and leave it in players hand.
I saw it as event decoration on Atlantic.
Edit：i don't know if it's still available, go south a few screens away from south west bridge in Britain on Atlantic.
You will find about 4 small house. Flaming skull is set inside of one of the 4.
Say Bauble in front of it, then you will get teleported safe stucking area, so be careful if you are newbie.

Halberd

(Qt. 2-4)

Rumor says these halberds were from event revenant. You could disarm it and loot.
Maybe EM didn't intended it occurred.
